The Center features a home-like environment with every modern convenience. Since it’s opening in 2010, the Center has been recognized throughout the world as an innovative model of care for seniors. And now, in 2020, we have been chosen to implement a new vent unit expansion! The unit will be equipped with State of the Art Technology, from the Vent Unit, to the resident having the ability to operate all aspects of their environment remotely. The candidate will be provided a structured education plan to assist in their growth and development on Vent Care. Continuing Education will be ongoing and at the forefront of this Vent House Expansion. We will be accepting eager nurses, willing to learn and have the desire for professional growth. The nurse is an essential care team member in The Green House model. (S)He is licensed to practice in the Commonwealth of Massachusetts. The Green House model is about building a human habitat where residents and staff are living rich and meaningful lives. At the same time, residents being served in The Green House homes have complex skilled nursing needs. Nurses need to assess and meet those clinical needs while supporting the quality of life of each resident. Nurses have multiple roles to fulfill to be effective in meeting the needs of the residents and staff in The Green House Home.
The Green House model is grounded in the notion that the residents have the right to excellent, evidence-based medical and clinical care. In the context of The Eden Alternative principles, however:
“Medical treatment should be the servant of genuine human caring, never its master.”
Quality of care is critical. But quality of life needs to be first in consideration of all care and treatment. It is the resident who determines what constitutes quality of life.
The nurse in The Green House home has the opportunity to practice in expanded professional roles to achieve person-directed care in a place called home. These roles are consistent with the rest of the Clinical Support Team, but because nurses are in the homes throughout the day they have closer and more immediate relationships with both Certified Nursing Assistant and residents.
CLINICAL SUPPORT TEAM (CST):
Nurses are core members of the Clinical Support Team (CST). Their role on the CST includes assessing and monitoring the clinical status of the residents and delivering evidence-base clinical care, both directly and through C.N.A.’s and other CST members. The CST nurses report directly to the Director of Nursing but work in collaboration with the Guide and C.N.A.’s.
DUTIES AND RESPONSIBILITIES:
- Obtain, communicate, and report on each resident within the interdisciplinary team.
- Visit each resident during each shift to monitor clinical status and to determine if the clinical care needs of the resident are being met. Clinical assessments should be resident specific and consider the primary diagnosis and risks specific to the individual resident.
- Assess each resident for unresolved/untreated pain. Plan and implement pain management interventions when appropriate.
- Assess, plan and implement interventions related to: skin integrity, bowel and bladder status, including continence and bowel movement monitoring, ambulation status, nutrition (weights), hydration and the effects of medications.
- Identify any clinical teaching needs of the C.N.A., resident or family. Provide teaching and complete documentation.
- Identify ways to support the C.N.A.’s in their clinical competency and skills.
- Monitor standard precautions (i.e.: presence of gloves, soap, towels, etc.).
- Administer treatments and medications.
- Perform an accurate respiratory assessment.
- Monitor overall adequacy of the care plan.
- Review compliance with the care plan.
- Maintain emergency supplies and equipment.
- Conduct physical assessment of each resident.
- Conduct MDS assessments, monitor infection control, and care planning activities.
- Participate in the care planning process.
- Familiarity and compliance with Nurse Practice Act.
- Graduate of a NLN approved school for professional nursing.
- Must possess a current, unencumbered, active license to practice as a Nurse in Massachusetts.
- CPR certified.
- Previous vent experience preferred but not required
- One year experience as professional nurse within last 3 years or completion of a refresher course in past year.
- Demonstrated skill in physical assessment.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to post-acute care.
- IV Therapy Skills desirable.
- Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies
