As elders age, lifestyles changes are inevitable That’s where we come in. The Chelsea Jewish Aging Life Care program helps seniors and their families explore the different types of support services before, during or after a crisis occurs. Our goal is for elders to live well while they age.

Care Plan Assessments

We develop individualized plans by assessing an older adult’s current level of functioning and examine his/her support systems and resources. There are local and state resources available, but they’re often difficult to find -- and to navigate. We can help sort through a myriad of topics, such as caregiving, financial concerns, safety, medical issues and social interaction. Let us put together a plan that’s right for your family.

Care Assistance

We support older adults and loved ones during the many phases of aging. For caregivers – whether close by or long-distance -- we provide short and long-term eldercare assistance. Our extensive knowledge of the resources within our community as well as the quality and costs of such services are a huge help to families.

Moving Assistance

Whether moving from one’s house, apartment, retirement complex, assisted living facility or nursing home, we provide the expertise and assistance to make a smooth transition. Let us simplify the decision-making process.

Our Care Professionals



Leslie C. Hazlett, LICSW, C-ASWCM

Leslie earned her Bachelor of Arts degree in Education and Human Development from The George Washington University and her Master of Social Work degree from Boston University. She completed post-graduate work in Case Management from Boston University and has been a Licensed Clinical Social Worker in the state of Massachusetts since 1993. She is a member of the National Association of Social Workers and the Aging Life Care Association (formerly the National Association of Professional Geriatric Care Managers).

For the past 24 years, Leslie has worked with older adults and their families in the community and health care settings. She was also the founder of a private Geriatric Care Management business in Marblehead, MA for 10 years. Leslie was instrumental in the development of the Aging Life Care Department within the Chelsea Jewish organization. She specializes in providing client evaluations, individual and family counseling, referrals to community resources, and coordinating discharge planning services. Additionally, Leslie serves as a Field Education Specialist and an Adjunct Professor at The School of Social Work at Salem State University.



Janice E. Glick, LICSW, C-ASWCM

Janice has held the role of Director of Social Work Services/Case Management at the Brudnick Center for Living for the past 17 years. She earned her Bachelor of Arts degree in Psychology from the University of Massachusetts and her Master of Social Work degree from Simmons College. Janice also has a specialized certificate as an Advanced Social Work Case Manager. She is a Licensed Clinical Social Worker in the state of Massachusetts and a member of the National Association of Social Workers and the Aging Life Care Association (formerly the National Association of Professional Geriatric Care Managers).

In her 25 years of experience, Janice gained insight into the inner workings of various hospital settings, outpatient mental health clinics, and skilled nursing facilities. Her experience in case management and geriatric psychiatry expanded her expertise to include the collaboration of care and helping families to navigate the difficult healthcare maze. Janice’s strong skill set and desire to help others led her to develop the Aging Life Care division within the Brudnick Center for Living. Additionally, Janice is an Adjunct Professor at Salem State University and serves as a field instructor for social work interns who are attending graduate schools of social work in the Boston area.
